A Case of Nodular Goiter with Follicular Carcinoma




Abstract
A biopsy case of nodular goiter with follicular adenocarcinoma is to be reported and a review of literature related to this case is presented in the aspects of etiology, incidence of carcinoma in nodular goiter and histopathology.
The patient, a school girl aged fifteen, was admitted to a local clinic because of a nontender, freely movable, multinodular mass on the anterior aspect of the neck and consequent pressure symptoms. During the previous 2 years she had a slowly growing mass in the thyroid gland without and- systemic symptom, but occasionally it showed a slight decrease in size. Just before admission she experienced momentary acute pain with sudden expansion of the mass. On December 1, 1967, a total th, roidectomy was performed at the local clinic under the diagnosis of nontoxic nodular goiter.
We received the totally resected thyroid gland and examined grossly and microscopically. The thyroid gland showed multinodular character and the intact capsule. On cut surface, many variegated nodules surrounded by a dense fibrous tissue was presented. The nodules varied from 0. 7 cm. to 2. 5 cm. in size, from fluctuant to relatively hard in consistence, and from gray-white to red to dark re in color. Cystic degeneration was present in a few nodules and some had hemorrhagic areas.
Histologically, the gland showed changes characteristic of nodular goiter, but a nodule, which was grossly gray-white and relatively hard, showed features of follicular adenocarcinoma, invading the surrounding fibrous tissue and the adjacent nodule.
